Your First Appointment

Completion of Forms: Please click on appropriate form (s) below, fill out in its entirety and email to OIMEETH@northwell.edu or print out and bring with you to your scheduled appointment. This process will ensure a seamless transition for your visit.

If you are unable to complete our forms prior to your first visit, please arrive 15-20 minutes prior to your appointment time to complete them in our office.
